An Introduction to DBT Primer Therapy

Dialectical behavior therapy (DBT) is an effective treatment for many conditions. It builds upon cognitive behavioral therapy (CBT), but has been specifically tailored to assist those who struggle with intense emotions and difficulty controlling them.

Although DBT has shown promising results in several controlled treatment trials, it is still not widely used as a standard practice within community mental health care. Nonetheless, increasing numbers of public mental health authorities recognize its potential benefits and require agencies looking to implement it form teams of clinicians and administrators before embarking on intensive training.

Teams practicing DBT are exposed to its five essential functions in an intensive series of team training workshops. During the initial five days, learners gain knowledge through didactic methods, experiential exercises and role playing exercises. Following that period of six months, practitioners practice DBT through extensive homework assignments, prepare for examinations on their programs and cases, and consult with other team members on existing problems.

This program provides a progressive learning process that leads to competence in DBT practice and adhering to the DBT model. Furthermore, individual therapists and group skills trainers must form consultation teams that meet weekly for ongoing education, support, supervision, and renewal.

DBT provides immediate practical help, but is built upon a sound model that offers long-term recovery-oriented treatment plans. Furthermore, this model is theory-driven rather than protocol-driven so it can be tailored to suit the individual needs of clients.

DBT draws upon the strengths of CBT and other approaches, providing a framework for treating borderline personality disorder and other complex psychiatric disorders. It teaches four major skills that promote emotional regulation, interpersonal effectiveness, distress tolerance, and mindfulness.

DBT can be effective for treating a variety of conditions, but it has particularly proven successful in treating those suffering from PTSD, self-injury and suicide. It has also been adapted to address other common treatment barriers like attachment issues, substance abuse issues and managing stress.

DBT emphasizes a hierarchy of therapeutic targets to address the most significant and harmful behaviors the consumer or therapist might be exhibiting, using techniques tailored specifically for that situation. For instance, in a treatment session, an initial priority might be identifying any recent or current suicide attempts by the client and devising strategies to prevent further attempts.

In the same session, strategies are employed to help the client manage feelings of anger, frustration or sadness. The therapist encourages them to take ownership over their responses and explore new methods for controlling emotions.

Many consumers may be reluctant to enter DBT due to concerns about the commitment required. Therapists make a point of explaining that their commitment is voluntary and provide alternatives so that people can decide if DBT is suitable for them.
