MADD (Mixed Anxiety and Depression Disorder)

Symptoms of anxiety and depression can interfere with daily activities. They cause emotional distress, impair daily functioning and increase the risk of suicide. MADD (Mixed Anxiety and Depression Disorder) is a subsyndromal psychiatric disorder, defined by symptoms of anxiety and depression occurring more days than not and that meet ICD-10 criteria. Early recognition and treatment can prevent the development of more severe affective disorders, which can save patients’ suffering and healthcare resources.

In order to diagnose MADD, subjects must meet the following criteria: (1) a combination of at least three or four major depressive symptoms and one or more anxiety symptoms; and (2) autonomic symptoms. For a diagnosis to be made, the CIS-R overall score must be greater than 12. CIS-R is a standardized measure of the severity of psychiatric morbidity.

The most recent version of the International Classification of Diseases (ICD-10), which was published in 1990, includes MADD in the section on anxiety disorders. However, MADD has not yet been included in the DSM-5. This is due to controversy over the inclusion of the disorder.

A review of studies on mixed anxiety and depression shows that most studies do not report outcomes for minor distress. Therefore, it is difficult to draw conclusions about the validity of MADD. Nevertheless, a number of studies have established the reliability and validity of a diagnostic criteria for this disorder.

One study looked at data from a school-based sample of 706 adolescents. Eighty-five percent of the subjects had significant anxiety symptoms and depression symptoms. Two factors were found to be significantly associated with anxiety and depression symptoms: the ‘Depression’ class and ‘Anxiety’ class.
