Using Neuromodulation to Prevent Migraines

Migraines are severe headaches that cause throbbing, pulsating pain on one side of the head. Additionally, they may cause nausea and vomiting, with some people experiencing a visual aura which appears up to an hour before an attack.

Treatment for migraines typically involves medications. Unfortunately, these may not be effective for everyone or may have unwanted side effects.

In an effort to discover a cure for migraines, researchers are exploring methods that alter brain activity and block pain signals – this research field being known as neuromodulation.

A transcranial direct current (tDCS) device sends electrical pulses to the back of the head that disrupt pain-sensing nerves in the brain, relieving migraine attacks and decreasing medication usage.

tDCS (total DC stimulation) is a noninvasive neuromodulation technique that has been demonstrated to reduce analgesic drug intake with few adverse events. This portable device has also been studied for migraine prevention purposes.

This device utilizes a handheld coil to deliver transcranial magnetic stimulation (TMS) to the back of the brain. TMS can reduce cortical spreading depression (CSD, an activating wave that triggers cells on the surface of the brain and is responsible for migraine aura), as well as thalamocortical signaling.

TMS can depolarize neurons in the occipital lobes that generate visual auras, believed to be responsible for most migraines. By using an elliptical coil that mimics human head curvature, this device stimulates both left and right occipital lobes simultaneously to eliminate auras from patients’ experiences.

Other devices used in bioelectric therapy for migraines include a portable TMS-based unit placed on the arm and a handheld device that can deliver an electric shock to the back of the head. These can be worn at the start of a migraine pain episode for up to 45 minutes or more and controlled through a smartphone app.

Another type of dds bioelectric therapy for migraines is GammaCore, an FDA-approved device that sends an electric signal to your vagus nerve (which runs along the sides of your neck). This device sends an electric signal to this nerve which has been proven effective at both preventing and treating migraines.

GammaCore is a portable hand-held unit that sends an electrical signal to your vagus nerve, located along the sides of your neck. This device can suppress cortical spreading depression (CSD, which causes cells on the surface of the brain and is responsible for migraine aura) as well as thalamocortical pain signaling.

This device utilizes a proprietary stimulation signal that targets specific pain transmitting nerve fibers, disrupting electrical activity in migraine headaches from a remote location (on the arm). It’s safe to use, costs $499, and includes three electrodes.

Noninvasive neuromodulation offers an attractive solution for patients who are having trouble tolerating current therapeutics, aren’t responding to prevention efforts, or want a way to reduce acute medication use. Although results take time to manifest, neuromodulation has the potential to be effective both during acute situations or as a primary preventive strategy in those with difficulty managing medications or limited choices of drugs.
